The following forms are necessary for students participating in school sports:

Parental Consent Form - Your "Athletic Parental Consent Form" required prior to the start of each sport season

Specialized sport's forms are needed for each sport. PLEASE CALL US if you have any questions.

The NJ Department of Education requires the ANNUAL ATHLETIC PRE-PARTICIPATION forms. There are TWO PARTS to this form:

Part "A" - Parents - Must complete this HEALTH HISTORY before EACH sport

Part "B" - Physician - Must complete this PHYSICAL EXAMINATION FORM once every 365 days.
